Skilled Nursing Services
Each P&N Vision Health Care client receives a personal in-depth assessment to our Director of Nursing that, among other things, takes the account of their condition, personality and environment. We then put that knowledge to use by creating an individualized plan of care that meets our clients' unique and physical needs -- a plan that strives to not only improve their health, but enhance their quality of life. This process, along with continual oversight by our nursing staff helps us ensure the right fit between our clients and their caregivers, and enables us to quickly modify client's plan of care as their needs evolve.
Daily Living Assistance
Our clinically based model of care provides full client oversight. This includes the following
• clinical and environmental assessments,
• supervision of our RNs, LPNs and CNAs, and
• comprehensive case management
to make sure you and your loved ones receive the highest quality of care.
P&N Vision Health Care also works hand in hand with physicians, referral sources and other health care professionals to ensure continuity and coordination of care for the best possible outcomes for our clients.
